This is an interesting topic, I am assuming for the purpose of this list we are looking at how the game plays not just the title.
So for example, look at strider, we have an nes game called strider, a modern update with the same name, and the genesis game.
The genesis game is a straight port of the arcade so that is not an exclusive.
The nes game has the same name but is a completely different game, so i would count that.
The update is also a completely different game but was released on multiple platforms so that would not count. Is that a correct way of classifying these?
How do we tackle a game like punch out? While punch out has been re released virtually, Mike Tysons punch out never has, even though it is essentially the same game would mtpo count as an exclusive?
How about games that are close but have exclusive features or something that makes them feel completely different? So for example, would stuff like new play control Pikmin and resident evil 4 wii edition count as exclusive? Thought they are basically ports the game play experience is completely different due to motion controls.
